London North West University Healthcare NHS Trust welcomes applications from people who wish to work part time or flexibly.

This is a newly comissioned service looking after patients identified as high risk of admission to hospital or readmission, providing high level clinical care led by Elderly care consultant in patients own home.

This will include working closely with GPs, WSIC, social care and community teams.

For this post, the following are essential:

  • NMC or HCPC Registration
  • Broad range of experience and excellent communication skills
  • Car driver/ owner (the Trust has a lease car facility available).


Integrated Care team  provides Nursing, Occupational Therapy, Physiotherapy and Dietitian services which can include specialist assessment, equipment and short-term packages of care.


We require Specialist Nurses with experience in a wide variety of clinical areas including the community setting. You will need to be able to demonstrate excellent clinical reasoning, risk assessment, verbal and written communication skills. Due to the response standards for the team you must be able to work under pressure and provide timely specialist nurse assessments and interventions. You will be expected to be able to work autonomously and undertake visits in line with the STARRS’ Lone Working Policy.


The team is operational 7 days a week between the hours of 08:00 and 17:00
