London North West University Healthcare NHS Trust welcomes applications from people who wish to work part time or flexibly.

An excellent and rewarding opportunity is available for an experienced and dynamic Allied Health Professional or Nurse, who is seeking a role which will utilise their substantial clinical experience/expertise to further develop their skills. The successful candidate will be responsible for identifying and coordinating the Stroke Care Pathway for all adult stroke patients. They will work closely with professionals across the pathway, from Hyper-Acute and Acute Stroke Units, Rehabilitation Centres, Brent Early Supported Discharge Team, Brent Rehab and Reablement Team and Social Services. Their key role will be to ensure that patients receive seamless care on admission through to discharge from the acute and rehabilitation phase and their return into the community. The successful candidate will be responsible for carrying six month reviews with stroke survivors and entering and managing the necessary data onto the stroke national audit programme – SSNAP (Stroke Sentinel National Audit Programme).

STARRS is a fresh approach to Intermediate Care working from the hospital setting to the community incorporating a Single Point of Access, a clinically focused Rapid Response Service working across A&E and community organisations. Another component of the service includes Early Discharge Support from in-patient settings and a Rehabilitation Service in the community.
